Feedback, Concerns & Grievances
At Supportive Community Innovations (SCI), we believe that open communication, mutual respect, and a willingness to listen are essential to providing high-quality, person-centered services and maintaining a supportive organizational culture.
We welcome feedback from everyone connected to SCI, including individuals receiving services, parents, guardians, caregivers, employees, schools, community partners, other professionals, and members of the public. Whether your experience involves our ABA Therapy, Medicaid Waiver Services, Counseling, or other organizational activities, we want to hear from you.
Your feedback matters. We welcome positive experiences, suggestions for improvement, questions, concerns, complaints, and formal grievances. We recognize that feedback helps us celebrate what is working well, identify areas that need attention, strengthen relationships, and continually improve our services and practices.
At SCI, we value open, respectful communication and encourage individuals to address questions and concerns through direct conversation and collaborative problem-solving whenever appropriate. We also recognize the importance of providing multiple ways to share feedback and raise concerns so that everyone feels comfortable being heard.
SCI is committed to reviewing concerns fairly, respectfully, and in a timely manner. Individuals have the right to raise concerns or submit formal grievances without fear of retaliation.
You may use the form below to share feedback about any SCI service, program, employee, or location. Submissions may be made anonymously, although providing contact information allows us to follow up with you directly.
Thank you for helping us continue to learn, grow, and strengthen the services and communities we support.
